Our in-home care services are built on the foundation of PeaceHealth’s decades of experience providing transformative health care -- bringing the tools, resources, caregivers, and commitment necessary to improve quality of life and comfort for our patients and deliver the highest-quality care to them each day.

Home and Community’s services are frequently sought or required following a disability, illness, injury, surgery or hospitalization. Care is delivered by PeaceHealth providers, nurses, therapists, medical social workers and other clinicians who are skilled in at-home care. Our team will help you recover or receive care in the place that’s comfortable for you.

Home Health

Individualized home health care is available to help patients recover from illness, injury or surgery, to avoid unnecessary rehospitalization and manage long-term conditions in the home. Skilled nursing, physical therapy, occupational therapy and speech therapy can be coordinated by your physician and other healthcare providers to develop a plan for you to manage medications and teach you about your condition.

Hospice

Hospice is a special program of compassionate care and support for terminally ill patients and their families. Care is delivered in homes, senior living communities, skilled nursing facilities or a hospice house. Hospice care is provided by a team of professionals and trained volunteers including registered nurses, home health aides, a medical director, a medical social worker, and a chaplain.

Palliative Care

PeaceHealth Palliative Care is specialized medical care with a focus on pain, symptom and stress relief for patients with serious illnesses. Palliative care is provided by a team of doctors, nurses, social workers, medical assistants and other specialists who work together with a patient’s other medical providers to provide an extra layer of support.

Advance Care Planning

Sharing your wishes for the care you want if you are critically ill can bring you closer to the people you love. Making your wishes known clearly in advance of a crisis is one of the biggest gifts you can give yourself, your family and friends. Our Advance Care Planning team provides resources and assistance to help you get your advance care planning documents completed – i.e., advance directive and Physician Orders for Life Sustaining Treatment (POLST) – for Washington, Oregon and Alaska!
