We work as a team with other Sacred Heart Medical Center personnel and community providers in providing comprehensive health care planning on behalf of the patient.
We also assist you and your family in identifying their needs and helping them find the best solutions available. Our services include helping you and your family to make the best use of available medical, emotional, interpersonal and community resources.

A Medical Social Work Care Manager collaborates with individuals, their families and the interdisciplinary team to identify patient needs. Our team of care managers determines an individual’s current capacity and support to develop a safe, supportive, and clinically appropriate plan of care. This plan can encompass family, friends, financial, spiritual and environmental care.
Our managers help coordinate the use of resources for positive health, patient and family satisfaction and financial outcomes. Our team also specializes in assisting patients with complex psychosocial issues. We work together with other departments, community agencies and providers to create coordinated and supportive discharge plans.
