Carolyn Wong, PharmD, BCPS, BCACP

Residency Program Director

Carolyn received her PharmD from Washington State University.  She completed a PGY1 Pharmacy Practice residency at University of Colorado Health Sciences Center and completed a PGY2 in Ambulatory Care at the University of Washington/Harborview Medical Center.  Her clinical interests include diabetes management, cardiovascular risk reduction, pain management, and advancing the practice of ambulatory care pharmacy.  In her free time, Carolyn loves traveling the world, discovering as many of the good eats and drinks in the area she can, working up a sweat hiking or doing something active, and cheering on the Blazers, Timbers, and her Cougs.

Residency Program Coordinator

Luke received his PharmD from the University of Colorado Skaggs School of Pharmacy. He completed a PGY1 Pharmacy Practice residency at the Ohio State University Wexner Medical Center and completed a PGY2 in Ambulatory Care at the University of Washington Harborview Medical Center Family Medicine Clinic. His clinical interests include pharmacist involvement in the management of diabetes, HIV, Hepatitis C, and COPD as well as advancing pharmacy practice into new areas, clinical research, and evidence-based medicine. In his free time, Luke enjoys hiking in the beautiful Pacific Northwest, brewing beer, and is working on getting his private pilot’s license.

PGY2 Resident 2022

Jackie received her PharmD from University of Washington School of Pharmacy.  She completed a PGY1 Pharmacy Practice residency with Providence Health & Services at Providence Portland Medical Center. Her clinical interests include chronic disease state management, smoking cessation, anticoagulation, geriatrics, deprescribing, and heart failure.  In her free time, Jackie enjoys playing with her French Bulldog puppy Lola, binge watching shows on any streaming platform, and going out to happy hour with friends. Upon completion of her PGY2 residency Jackie stayed on with us at PeaceHealth Southwest/Family Medicine of Southwest Washington.

Chloe Nguyen, PharmD

Chloe received her PharmD from Oregon State University/Oregon Health and Science University College of Pharmacy. She completed her PGY1 Pharmacy Practice residency at Providence Medford Medical Center and completed her PGY2 in Ambulatory Care at Providence Medical Group - Southern Oregon. Her clinical interests include maangement of diabetes, heart failure, women's health, as well as population health. In her free time, Chloe enjoys hiking, traveling, and spending time with family.

Lydia Niou, PharmD, BCPS, BCACP

Lydia received her PharmD from The University of Washington and completed her PGY1 residency at PeaceHealth Southwest Medical Center. Her clinical interest include diabetes, anticoagulation, and geriatrics, as well as helping patients afford their medications. In her free time she enjoys spending time with her family, baking, reading, board games, and planning future trips.

Austin Horner, PharmD

Austin received his PharmD from the University of Kansas School of Pharmacy. He has been a manager/PIC most of his career. His clinical interests include Transitions of Care, data utilization, and billing and coding to help advance the practice of ambulatory care pharmacy. In his free time, Austin enjoys getting out into mother nature, trivia games, and spending time with his wife and 3 children.

Laura received her PharmD degree from Pacific University School of Pharmacy in Hillsboro, Oregon. She completed a PGY1 Pharmacy Practice residency at Providence Portland Medical Center and a PGY2 in Ambulatory Care at Providence Medical Group in Portland, Oregon. Her clinical interests include diabetes, heart failure, population health and transitions of care.  In her free time, Laura loves baking delicious desserts, reading a good book, traveling anywhere tropical and camping in the Pacific Northwest.
