Refer a Patient to Oregon Heart & Vascular Institute (OHVI) RiverBend Nuclear Medicine

Patients may be referred to OHVI RiverBend Nuclear Medicine for a variety of diagnostic imaging studies. Referrals help ensure patients receive the appropriate exam, preparation instructions, and timely scheduling. For a complete list of imaging services, please view the scheduling request order form.

All nuclear medicine exams require a valid provider order prior to scheduling.

Information for Referring Providers

To refer a patient for a nuclear medicine exam: 

  1. Download and complete the Nuclear Medicine order form
  2. Include all required clinical and insurance information
  3. Fax the completed order to the number listed on the form

Note: Incomplete orders or missing information may delay scheduling.

Information for Patients

If your provider has referred you for a nuclear medicine exam:

  • Our department will contact you after your referral is received
  • You do not need to call to schedule unless instructed
  • Preparation instructions will be provided before your appointment
