Welcome to the PeaceHealth Oregon Medical Staff Services' provider credentialing web site. Properly credentialed providers may see patients at any PeaceHealth facility in Oregon.

To be eligible to apply for initial appointment or reappointment to the PeaceHealth Oregon Medical Staff, physicians, dentists and podiatrists must:

  • Have a current or pending unrestricted license to practice in Oregon and have never had a license to practice revoked or suspended by any state licensing agency
  • Where applicable to their practice, have a current, unrestricted DEA registration
  • Be located (office and/or residence) within the geographic service area of the Medical Center, as defined by the Board, close enough to fulfill their Medical Staff responsibilities and to provide timely and continuous care for their patients in the Medical Center
  • Have current, valid professional liability insurance coverage in a form and in amounts satisfactory to the Medical Center
  • Since medical school:
    • Have not been convicted of Medicare, Medicaid, or other federal or state governmental or private third-party payer fraud or program abuse, nor have been required to pay civil penalties for the same
    • Have not been, and are not currently, involuntarily excluded or precluded from participation in Medicare, Medicaid, or other federal or state governmental health care programs
    • Have not had Medical Staff appointment or clinical privileges denied, revoked or terminated by any health care facility or health plan for reasons related to clinical competence or professional conduct, and have never resigned appointment or relinquished privileges during a Medical Staff investigation or in exchange for not conducting such an investigation
    • Have not been convicted of, or entered a plea of guilty or no contest to, any felony or any misdemeanor (including any conviction or plea that may have been expunged) relating to controlled substances, alcohol, illegal drugs, insurance or health care fraud or abuse, or violence
  • Agree to fulfill all responsibilities regarding emergency call
  • Have coverage arrangements with other appointees to the Medical Staff holding similar clinical privileges for those times when the individual shall be unavailable
  • Have successfully completed a residency training program approved by the Accreditation Council for Graduate Medical Education, the AOA, or the Royal College of Physicians and Surgeons of Canada in the specialty in which the applicant seeks clinical privileges, or an oral and maxillofacial surgery training program accredited by the Commission on Dental Accreditation of the American Dental Association, or a podiatric surgical residency program accredited by the Council on Podiatric Medical Education of the American Podiatric Medical Association (This requirement is applicable to only those individuals who apply for initial staff appointment on or after the date of adoption of this Policy.)
  • Are board certified in their primary area of practice at the Medical Center. Those applicants who are not board certified at the time of application but who have completed their residency or fellowship training within the last five years shall be eligible for Medical Staff appointment. However, in order to remain eligible, those applicants must achieve board certification in their primary area of practice within five years from the date of completion of their residency or fellowship training (This requirement is applicable only to those individuals who apply for initial staff appointment after the date of adoption of this Policy. All individuals appointed previously shall be governed by the board certification requirements in effect at the time of their appointments.)
  • Maintain board certification and, to the extent required by the applicable specialty/subspecialty board, satisfy recertification requirements
  • Demonstrate recent active clinical practice during at least two of the last four years
  • If applying to practice in a specialty covered by an exclusive contract, have a contractual arrangement with the group or individual holding the exclusive contract
  • Be appointed to the Medical Staff of PeaceHealth Sacred Heart Medical Center, University District, in any staff category
