Preventing Clots in Chaos: VTE Prophylaxis in Trauma Care

Objectives:
- Describe the pathophysiology and risk factors for VTE in trauma patients, including how injury severity, immobility, and inflammatory responses contribute to clot formation.
- Compare mechanical and pharmacologic prophylaxis strategies, including indications, contraindications, and timing for initiation in various trauma scenarios (e.g., TBI, spinal injury, solid organ injury).
- Apply weight-based enoxaparin dosing protocols, including when to monitor anti-Xa levels and how to adjust dosing for obesity, renal impairment, or bleeding risk.
- Interpret current guidelines and algorithms from leading trauma organizations (e.g., WTA, Vanderbilt, ASVE Trauma) to make evidence-based decisions about VTE prophylaxis timing and duration.
- Develop a post-discharge VTE prevention plan for high-risk trauma patients, including criteria for extended prophylaxis and patient education strategies.
