Home and Community Southwest provides the professional and technical excellence of Southwest in the comfort of your home. It's the remedy for healthcare needs in many situations.

Whether you are being discharged from the hospital or being treated at home for an illness, learning how to manage a new medical condition, recuperating from surgery, or assisting a family member or friend with healthcare, our services are available to assist you.

Availability

We can schedule visits between 8:00 a.m. and 4:30 p.m., Monday - Friday
A Nurse is on call 24 hours per day.

Services

HomeCare Southwest's services include:

• Nursing Care provided by registered nurses who are experienced in medical, surgical, general and psychiatric care.
• Rehabilitation provided by physical, occupational, and speech therapists experienced in rehabilitation treatment, as well as home safety and adaptations.
• Social Services provided by medical social workers who are experienced with treatment and resources to address personal issues resulting from your illness which impact your recovery or recuperation.
• Home Health Aides who can do or assist with personal care and hygiene, such as bathing, shampooing, etc. Home health aides also can assist the nurse or therapist with treatments or exercises.
