At PeaceHealth, our Mission calls us to continually strengthen the way we care for our patients and communities — especially as needs grow and the healthcare landscape continues to change. As part of this ongoing work, we are making thoughtful updates to our Care Management structure, including changes to how our Advanced Care Management Planning is administered.
This change reflects our responsibility to build a sustainable care model — one that allows us to meet increasing patient needs, operate more efficiently and ensure the right support is integrated seamlessly into the care experience.
Integrating Advance Care Planning into everyday care
Advance care planning is an essential part of empowering patients to make informed decisions about their health and future. Advanced Care Planning conversations and related documentation are incorporated directly into the routine, compassionate care we provide every day.
This approach means:
- All members of the care team — from physicians to clinicians to medical assistants —play a role in supporting patients through these discussions.
- We’ve prepared caregivers and physicians to have these conversations with clarity and sensitivity, through education and ongoing access to tools and resources.
- Whether someone is facing a new diagnosis, considering treatment options, or planning ahead, caregivers will be prepared to talk through preferences in a way that fits naturally into the flow of care.
By integrating these conversations into regular appointments, patients can feel more supported, heard and respected — without needing to schedule a separate class or visit.
