PeaceHealth Clinically Integrated Network (CIN)
Value-Based Care Vision
The vision for value-based care is:
- Growing differently to provide convenient, accessible and high-quality care.
- Thinking differently to adopt a wellness-first, population health mindset.
- Providing a different patient experience through digital technologies and data insights.
- Enabling safe, seamless and highly personalized experiences.
About CIN
A clinically integrated network (CIN) is a group of providers, hospitals and clinics that work together to provide high-quality care and reduce unnecessary costs to specific populations within defined communities.
A CIN uses clinical governance and quality metrics to create a unified regional healthcare delivery system that demonstrates value while enhancing patient care and experience.

Community Connect
Electronic Health Record (EHR) and practice management systems are a “must-have” for medical practices to effectively deliver day-to-day patient care, promote optimal health maintenance practices and fulfill required regulatory reporting.
- Community Connect is an extension of the EHR system (and associated support required) that PeaceHealth hospitals and clinics have been effectively using for nearly 10 years.
- The independent practice EHR is geared toward independent and small-group medical practices that might find the acquisition of a similarly comprehensive system out of reach, logistically and/or financially.
Community Connect:
- Supports start-to-finish operational processes supporting patient care — from scheduling and clinical documentation to revenue cycle management and reporting.
- Keeps your office’s patient records safe and secure.
- Stays current on proven advances in technology and regulatory requirements.
- Gives patients tools and access to view their records and interact with their provider.
- Places best-practice clinical decision support tools at your fingertips.
Why build or buy your own when you can affordably access a comprehensive system and all associated support from a trusted source in your community?
