There’s a lot of talk these days about self-care. But when you have one or more chronic health conditions, it can feel overwhelming to manage them all by yourself.  

That’s where PeaceHealth Flourish comes in. You get one-on-one support from a whole team who can help you understand and keep up with your health plan(s). Bottom line, it means helping you take the steps to feel your best.  

We’ve seen firsthand the difference that this type of support makes. People who join Flourish are up to 30% less likely to go to the hospital or the emergency room because of their health conditions.

Why join Flourish?

Flourish meets you where you are — without judgment. We’re here to help you learn how to take care of your health today and every day.

Medicare and insurance covers most of the cost for Flourish. If you need help with co-pays or don’t have insurance, PeaceHealth offers financial assistance.

Above all, you don’t have to figure things out for yourself!

Our team joins with you and your primary care provider to set reachable health goals.

What goals? It depends on the condition(s) you’re trying to manage. One goal might be to be more physically active. Another might be to keep your blood sugar at target levels. Or maybe you want to lower your cholesterol.  

Together, we’ll work with you to outline steps that will help you feel your best. And we’ll stay with you for the next year, supporting you on the way to better health. 

You’ll have a care manager, usually a nurse or a social worker. This person is dedicated to:  

  • Helping you learn how to manage your health.
  • Coordinating your care, such as help setting appointments or getting prescriptions filled.  
  • Matching you with the services and resources that could help you even more.  
  • Checking in every month to see how you’re doing.

Services you’ll have access to include:  

  • Referrals to doctors who specialize in your condition(s).  
  • Help with food, housing and transportation.  
  • Access to medical supplies you may need.  
  • Support for mental health or substance-use concerns.
