- Prior to scheduling an appointment: If you have a fever, cough or other symptoms of possible infection, please notify us when you schedule an appointment and before you arrive. If you’re having a medical emergency, call 911.
- Bring a mask: For everyone's safety, masks are required in all PeaceHealth facilities.
- Entrance screening: Upon arrival, everyone will receive a symptom and temperature check. Anyone with an elevated temperature will be connected with appropriate care.
- Visitor restrictions: No visitors are allowed in clinics except for those accompanying patients needing assistance.

From I-5: Exit 2 39th Street/Highway 500. Head west from exit to 39th St. Turn left from onto 39th Street. Turn left at Main Street (the first light after the I-5).
From I-205: Exit 1 Hwy 14 West to Vancouver. Exit to Interstate 5 North. Exit 2 -- 39th Street/Highway 500. Head west from exit to 39th Street. Turn left from exit onto 39th Street. Turn left at Main Street (the first light after the I-5).
