Rights and Responsibilities of Patients

Your well-being is our primary concern. While you are a patient, your basic right to make decisions, ask questions and be treated as an individual is as important to you as the quality of your medical care.

Rights of Patients

As a patient, you have:
  1. The right to impartial access to medically indicated services available at Sacred Heart Medical Center.
  2. The right to considerate, respectful care.
  3. The right to know the names of doctors caring for you and which doctor is coordinating your care.
  4. The right to receive, from your doctor, information concerning your care and condition in terms you can understand, and information necessary to give informed consent before the start of any procedure. Except in emergencies, this information usually includes a description of the procedure, significant risks involved, how long you may be incapacitated and reasonable medical alternatives.
  5. The right to consult with another doctor at your own request and expense.
  6. The right to contact the department manager if you have concerns with the care you received while hospitalized.
  7. The right to refuse treatment and to be informed of the medical consequences of your refusal.
  8. The right under Oregon law to prepare and submit a Directive to Physician or Power of Attorney for Health Care, which are commonly known as Advance Directives.
  9. The right to privacy in the performance of your medical care.
  10. The right to confidentiality of records and communications regarding your care.
  11. The right to give or withhold your consent to participate in research projects or procedures.
  12. The right to examine and receive an explanation of your hospital bill.
  13. The right to have a family member or representative of your choice and your own physician notified promptly of your admission to the hospital.
  14. The right to receive care in a safe setting.
  15. The right to be free from all forms of abuse and harassment.
  16. The right to access information contained in your clinical records within a reasonable time frame.
  17. The right to have concerns about your care addressed. Most concerns you may have while hospitalized may be easily addressed by the physician or nurse caring for you. If you would like additional help while hospitalized, after discharge or as an outpatient, please contact the manager of the nursing unit where you received services.
  18. The right to have his or her cultural, psychosocial, spiritual and personal values, beliefs and preferences respected.

Responsibilities of Patients

As a patient you have:

  1. The responsibility to participate actively in decisions regarding your health care.
  2. The responsibility to provide accurate, complete and timely information regarding your medical history, current symptoms and problems, and other matters relating to your health.
  3. The responsibility to ask questions and seek clarification in order to understand and be informed about your diagnosis and treatment, as well as what is expected of you.
  4. The responsibility to notify your doctor or nurse at once if you notice any changes in your health, you have any concerns about your care, or you cannot or will not follow a certain treatment plan.
  5. The responsibility to follow the instructions and advice of your doctor.
  6. The responsibility to be considerate of other patients and hospital personnel and to assist in the control of noise and number of visitors.
  7. The responsibility to be respectful of the property of other persons and of the hospital and to follow hospital rules affecting patient care and conduct.
