PeaceHealth Southwest Medical Center (PHSW) began offering a one-year PGY2 Ambulatory Care residency in July 2019 and was accredited through the American Society of Health-System Pharmacists (ASHP) in August 2020.

The purpose of this PGY2 ambulatory care pharmacy residency program is to build on Pharm.D. education and PGY1 pharmacy residency training to contribute to the development of clinical pharmacists in the specialized area of ambulatory care practice.  PGY2 residencies provide residents with opportunities to function independently as practitioners by conceptualizing and integrating accumulated experiences and knowledge and incorporating both into the provision of patient care.  Residents who successfully complete an accredited PGY2 ambulatory care pharmacy residency are prepared for advanced patient care, academic, or other specialized positions, along with board certification.

The resident will practice in a fully integrated interdisciplinary learning environment.  Washington state scope of practice allows pharmacists to become credentialed to bill for services and to practice under collaborative drug therapy agreements, which allow pharmacists to exercise prescriptive authority for a wide range of disease states with independent clinical decision making.

Types of pharmacist visits include:

  • Anticoagulation
  • COPD
  • Diabetes
  • Hepatitis C
  • HIV intake
  • Hypertension
  • Medicare Wellness Visits
  • Polypharmacy
  • Smoking cessation

The primary site of practice, PeaceHealth Family Medicine of Southwest Washington, is a community-based University of Washington School of Medicine-affiliated medical residency program.  The resident will be involved in medical resident education as well as PGY1 pharmacy resident education and precepting.
