Notice of unauthorized access of patient information

 
On Aug. 9, we discovered through patient privacy safeguards in place that a PeaceHealth Southwest Medical Center employee unnecessarily accessed electronic files containing protected health information for a group of patients.The employee responsible no longer works for PeaceHealth.

After conducting a thorough investigation, we concluded that unnecessary access occurred between November 2011 and July 2017. The information accessed did not include patients’ social security numbers or financial information.

Based on the nature of the information accessed, we do not believe affected patients are at risk for identity theft. We recommend, however, that affected patients regularly review the explanation of benefits received from their health insurers. If they show services that the patient did not receive, they should contact their insurer immediately.

PeaceHealth sincerely apologizes for this incident and any concern this may cause our patients. Patient privacy is among our highest priorities, and we take this very seriously. PeaceHealth began sending letters to affected patients on Monday, Sept. 11. If you believe you are affected and have not received a letter, please call 1-888-449-6634, Monday through Friday, 6 a.m. to 6 p.m. Pacific Time.

Consistent with our commitment the privacy of our patients, we continue to invest in advanced technology and follow industry best practices to appropriately monitor and safeguard protected health information of the people we serve. In addition, we are reinforcing education with our staff regarding the appropriate access of patient information.
