Notice of unauthorized disclosure of patient information


On Feb. 22, PeaceHealth discovered that a number of billing statements uploaded to the United States Bankruptcy Court’s e-filing system included some unredacted protected health information. The statements may have included names, addresses, medical account numbers, dates of service, procedures and names of insurance. The statements did not include patients’ social security numbers or other private financial information. Based on the nature of the information accessed, we do not believe affected patients are at risk for identity theft.

We sincerely apologize to our patients for any concern or inconvenience this may have caused. The privacy of our patients is among our highest priorities at PeaceHealth, and we take this very seriously.

After a thorough review, we concluded that patients who may have been impacted include those who were billed for services at PeaceHealth facilitates in Oregon and Washington between February 22, 2013, and February 22, 2019, and later filed for bankruptcy. PeaceHealth has sent letters to impacted patients and is in the process of redacting all relevant information with the bankruptcy courts. We have also changed our processes to prevent this instance from happening again.
