Systems Setup Process

Step 1: User Setup Request and Confidentiality Agreement

Please complete BOTH the User Setup Request and Confidentiality Agreement.

Please type your information into the boxes and click Print Form.

NOTE: PeaceHealth requires all of the information requested on the forms, including last 4 of Social Security Number (SSN), Employer Identification Number (EIN), and signatures from both the user and supervisor. Incomplete forms may be returned and will delay access.

Step 2: Fax Completed Forms

Fax the completed User Setup Request and Confidentiality Agreement to 360-729-3003.

EMR Security Change

This e-form is only to be used to modify existing EHR access, including removal of an inactivity block in CareConnect (Epic). It is not to be used to request a new system activation or a reactivation.
