Systems Setup Process

Step 1: User Setup Request and Confidentiality Agreement

Please complete BOTH the User Setup Request and Confidentiality Agreement.

Please type your information into the boxes and click Print Form.

NOTE: PeaceHealth requires all of the information requested on the forms, including last 4 of Social Security Number (SSN), Employer Identification Number (EIN), and signatures from both the user and supervisor. Incomplete forms may be returned and will delay access.

Step 2: Fax Completed Forms

Fax the completed User Setup Request and Confidentiality Agreement to 360-729-3003.

NOTE: Please wait 48 hours after submittal before inquiring about status. Allow three full business days for processing.

EMR Security Change

This electronic form is only to be used to modify existing active access to Centricity, Imagecast, CareConnect(Epic) or PH Link. It is not to be used to request a new activation or a reactivation of these systems.

Request for a provider to be added to Centricity Enterprise (CE) and Epic Ambulatory (CareConnect). This form is to be used for Providers NOT already in CE or CareConnect.
