What We Are Doing:
Nurses review all of your medications with you when you come to the hospital. These are entered into your medical record and then reviewed and confirmed by your physician. This list is maintained and updated with any changes during your stay. At discharge, the list is once again reviewed by the discharging provider. It's a part of your record that a PeaceHealth doctor can quickly review. If your provider is not part of PeaceHealth, this updated list can be faxed to any clinic, and wherever you go, we give you a copy.
