Living Will  

A living will is a legal document that you can use to make your wishes known regarding life-prolonging medical treatment. Other common names for a living will include advance directive, health care directive or a physician's directive.  

It is important to have a living will to inform your health care providers and your family about your desires for medical treatment in the event you are not able to speak for yourself.

Below are resources to help you complete an advance directive (living will):   
  • Five Wishes® booklets can be purchased for $2.50 in the gift shop at PeaceHealth​ St. Joseph Medical Center.  They are also available online for $5.00 from Aging with Dignity. (1-888-5WISHES) 
  • Visit the Washington State Medical Association’s end-of-life resource page for excellent information and downloadable forms to complete an advance directive on your own. 
  • The American Bar Association’s Commission on Law & Aging has put together a toolkit of helpful resources to help you plan your living will. Worksheets found on their site will help you discover, clarify and communicate what is important to you as you plan your living will.
