Individual Assessment
Clients are assessed for needs and given one-to-one attention in planning how they will make lifestyle changes. Our goal is to provide the information and support that allows clients to improve their self-management skills and confidence in their ability to achieve the best health they can.
Participants receive a blood glucose monitor, individual and group instruction, blood pressure, weight, and foot checks as needed.

Participants receive the Diabetes Self Management Program Manual. Classes provide interaction and skill practice for participants.

Diabetes Series
Monday Mornings: 8:30 a.m. - 10:30 p.m.
Monday Afternoons: 3:00 p.m. - 5:00 p.m.
Tuesdays Evenings: 6:00 p.m. - 8:00 p.m.
Flexible Insulin Class
Training for individuals on insulin and insulin pumps. Learn how to adjust your insulin for changes in activity and food choice that keep your blood sugar in control.
Referral is required. A support person is welcome to attend with you. Please call (360) 636-4943 to register.
Ongoing Follow-up and Support
Clients work individually with educators to achieve goals, overcome roadblocks, deal with health changes, and enhance self-management skills.

The Diabetes Self Management Program is recognized by the American Diabetes Association for providing education that meets National Standards for Diabetes Education.
