Your medical record is an important part of your personal history. Here at PeaceHealth we are committed to improving patient care, communication and safety through advanced healthcare technology. Throughout our facilities we have implemented an Electronic Medical Records (EMR) system. An EMR is a medical record that is stored digitally in a secure computer database. Healthcare is a complex and critical system, so it is important that your healthcare providers have immediate access to your medical records. Our EMR system also provides faster communication with other PeaceHealth providers on your care team, including those at PeaceHealth facilities outside of Florence. Your medical records are stored on a secure computer system and can only be accessed by your physician and other medical staff who need it to care for you. PeaceHealth strictly follows HIPAA guidelines for ensuring that all medical records are kept confidential. Patients may request their medical records at any time.

To obtain a copy of your records or have your medical records sent to another provider, ask for a health records authorization form from the Medical Records Department at either the hospital or clinics.
