Chronic Disease Self-Management Program
This is a free six-week program sponsored by the United Way of Lane County and the 100% Access Healthcare Initiative. It helps to create self-sufficiency among adults of any age with one or more chronic conditions.
Weekly workshops arm patients with skills to help manage medications, deal with fatigue and depression, manage symptoms, and more. Each participant receives a copy of the book, "Living a Healthy Life with Chronic Conditions," and a relaxation exercise on audiotape or CD.
This course is helpful for:
- Adults of any age who have one or more chronic conditions
- Family members or support persons who are welcome and encouraged to attend
- Managing medications
- Dealing with depression
- Eating well
- Meeting your goals
- Communicating better
- Starting an exercise program
- Managing stress and relaxation
- Fighting fatigue and frustration
- Solving problems
- Helping control pain

- At the last session participants who have attended at least five of the six sessions will receive a $20 gift card
