any of the following questions. | | Yes | Do you awaken from sleep frequently to urinate? | | Yes | Does pain frequently awaken you from sleep? | | Yes | Does the same medical problem frequently awaken you from
sleep? | | Yes | Has your sleep problem caused you to limit your usual
daily activities for longer than 2 weeks? | | Yes | Do you still have a sleep problem after 2 weeks of home
treatment? |
