School progress report
Home report cards are brief evaluations of a child's performance and behavior in the classroom, such as obeying requests, completing assignments, following instructions, asking for permission, or getting along with other children. At first, these may be completed by the teacher daily. A home report card may look like this:

Use this form to record my child's progress today in school performance and getting along with other children.
Rank my child's performance on a scale where 1 indicates not satisfactory, 2 indicates showing improvement, and 3 indicates satisfactory.
