This on-line resource describes the welfare benefit plans currently in effect for PeaceHealth benefit-eligible employees. Each section explains why these benefits are offered and highlights program components.
FlexAbility On-Line describes benefits in effect as of January, 2004. Specific features of a benefit plan may change from time to time. FlexAbility On-Line is a summary of your various benefit plans. This summary was written to provide you with an explanation of the main features of your benefits. Of course, this summary cannot cover every circumstance as it applies to you. The controlling provisions of the plan are in the master plan documents, and neither this summary plan description, nor any certificate may in any way modify these provisions. All benefits are administered according to plan documents. Every effort will be made to communicate plan changes to you in a timely manner.

The FlexAbility benefit plan is built on some basic
principles.
- To provide plans that cover a diverse group of employees
with a variety of lifestyle and family needs.
- To provide a competitive package of benefits that will
attract and retain talented, productive employees.
- To help employees protect against financial loss through
sickness and/or accident.
- To design plans that encourage employees to practice
preventive health care and work to maintain a healthy
lifestyle.
- To reduce health care and benefit costs.
FlexAbility offers you a choice of coverages including
medical, dental, vision, long-term disability, life and
accidental death and dismemberment (AD&D), and flexible
spending accounts for health care and day care expenses. Coverage
includes benefits for you, the employee, as well as your
dependents.
If you are covered under another medical plan, and can
submit proof of coverage, this benefit plan may be waived and a
portion of the credits returned to you.

Each October you will have the opportunity to make changes to
your benefits during a time called Open Enrollment. Keep in mind
when deciding on options during open enrollment that any changes
you make in October will take effect on January 1 of the
following year and remain in effect until December 31.

depending on your region and employment status. For detailed

Your Claims Administrator's employees observe
strict confidentiality when handling or processing your claims and records.
Violation of member confidentiality is grounds for dismissal.
By accepting the plan membership, you authorize
the Claims Administrators to release information in certain circumstances:
- The Claims Administrators have the right
to examine your medical records for any condition for which you claim
benefits as a member.
- The Claims Administrators have the right
to release member information to physicians, hospitals, and other
providers unless state or federal law prevents disclosure without your
consent.
- The Claims Administrators have the right
to use your medical records in order to conduct professional review
programs for health services.
We encourage you to use PeaceHealth facilities and resources.
When you consistently rely on our services, we can better
understand your health care needs and control costs to meet those
needs.
The FlexAbility plan administrators welcome your views and
recommendations regarding your benefit plans. We strive to
continuously improve our products and services. Your feedback is
essential to that effort.
