Measure: When a patient leaves the hospital's care, a complete and reconciled list of the patient's medications is provided directly to the patient and the patient's family as needed, and the list is explained to the patient and/or family.

What We Are Doing:

The process of medication reconciliation is an important part of the discharge process.  Your physician will review the medications you took prior to coming to the hospital and the medications you took while in the hospital.  He/she will then determine which medications you need to take once you go home.  This list will be reviewed with you and any questions you have will be answered.  A copy of the list will be given to you and your provider if he/she is not part of PeaceHealth.  Questions may be routed to: Barbara Bigelow, VP Quality at 907-821-1990.
