Measure: Manage all identified cases of unanticipated death or major permanent loss of function associated with health care associated infection as sentinel events.

What We Are Doing:

Our processes are regularly reviewed and improved wherever possible.  Should an unexpected adverse outcome occur, the healthcare team at PeaceHealth Ketchikan Medical Center immediately analyzes the clinical course using the "Root Cause Analysis" process.  This process helps to determine if any action or therapy could have been handled in a different manner and steps are put in place to help prevent a similar situation from happening again.  If the patient or her/his family is impacted by a "sentinel event", the Vice President for Patient Care discusses the event with the patient and family.  Questions can be routed to:  Barbara Bigelow, CHI Director at 907-821-1990.
