| Measure: Identify and review at least yearly a list of look-alike, sound-alike drugs used in the hospital and take action to prevent errors involving the interchange of these drugs. Time Period Ending: 12/31/2012 What We Are Doing: There are many thousands of drugs used today and it's a challenge to keep up with new names and shapes. Drugs that either sound or look alike can potentially be confused with one another. These Sound alike/Look alike drugs are reviewed by a team of pharmacists, doctors and other healthcare professionals every year. Then we educate staff about them, and build in steps, such as extra labeling and alerts to prevent errors. |
