Measure: Identify and review at least yearly a list of look-alike, sound-alike drugs used in the hospital and take action to prevent errors involving the interchange of these drugs.

What We Are Doing:
This measure is no longer a NPSG and has been incorporated into the Medication Management Standards.  Sound alike/Look alike drugs are reviewed by a team of pharmacists, doctors and other healthcare professionals every year. Then we educate staff about them, and build in steps, such as extra labeling and alerts to prevent errors.
