What We Are Doing:

Medicine today is complex and the amount of information used in your care can be overwhelming. In addition to communicating traditional systems assessments and the current plan of care we use the SBAR (Situation, Background, Assessment, Recommendation) Method at St. Joe's to to assure critical information is communicated during hand-offs between caregivers. These safe handoffs can occur in many ways: nurse-nurse, physician-physician, nurse-physician, physician-physical therapist; at the time of admission to a nursing unit, upon transfer to a different nursing unit, or after a surgical consult. Patient handoffs that utilize effective communication result in better and safer care.
