Measure: A list of "do not use" abbreviations, aconyms, symbols and dose desciptions is created and applied to all handwritten or free text orders or medication documents and to preprinted forms.

What We Are Doing:

PeaceHealth Ketchikan Medical Center, New Horizon's Transitional Care Center, our Home Health Care Department and our medical staff have a list of certain abbreviations, symbols, acronyms, and dosage descriptions that may lead to error in interpretation and cause patient harm.  "Do Not Use Abbreviations" lists are included in the patient's chart and in strategic locations throughout the hospital.  This national patient safety goal is monitored through audit and observation.  Staff are educated in new employee orientation, by the management leaders who expect the practice to occur, and through use of e-learning tools.  Questions may be routed to:  Barbara Bigelow, VP Quality at 907-821-1990.
