Measure: Prior to any specimen collection, medication administration, transfusion, or treatment, the hospital actively involves the patient, or the patient's family, to identify the patient using two unique identifiers.

What We Are Doing:

PeaceHealth Ketchikan Medical Center, New Horizon's Transitional Care Center and our Home Health Care Department all practice two patient identifiers.  This national patient safety goal is measured quarterly through audit and observation.  Staff are educated in new employee orientation, by the management leaders who expect the practice to occur, and through use of e-learning tools.  We encourage our patient's to challenge us by expecting this practice.  Questions can be routed to:  Barbara Bigelow, VP Quality at 907-821-1990.
