|Measure: Prior to any specimen collection, medication administration, transfusion, or treatment, the hospital actively involves the patient, or the patient's family, to identify the patient using two unique identifiers.|
Time Period Ending: 3/31/2013
What We Are Doing:
Use of Two Patient Identifiers. Our policy at Peace Harbor requires that nurses use at least two distinct ways to confirm who you are and what you are here for before any treatment. For example, having you spell your last name, state your date of birth and compare this information to your name bracelet. We review records regularly to ensure this is done every time.
