What We Are Doing:
We know that consistently teaching patients how to live with their heart failure has been proven to help keep them well.

• Our doctors, nurses, Cardiac Rehabilitation department and discharge planners begin educating patients about heart failure in the hospital and give them written instructions to take home.
• We update and educate our patients about their medications and send this information to their regular healthcare provider when they're discharged.
• Our new Planned Care team is now looking at ways to continue helping patients manage their own care at home.
• We now have six full and part-time cardiologists on staff, including heart failure specialists, helping to refine our program to provide care of the highest level.
• We've created a Transitional Care clinic that ensures patients are seen shortly after discharge to ensure they understand and can follow a treatment plan tailored to their needs.
• At PeaceHealth St. John Medical Center, we see our patients as partners in their healthcare. What our patients understand about medications, diet, and symptoms can make a big difference in helping them stay well.
