What We Are Doing:
We know that consistently teaching patients how to live with their heart failure has been proven to help keep them well.
Our doctors, nurses, Cardiac Rehabilitation department and discharge planners begin educating patients about heart failure in the hospital and give them written instructions to take home.
We update and educate our patients about their medications and send this information to their regular healthcare provider when they're discharged.
Our new Planned Care team is now looking at ways to continue helping patients manage their own care at home.
We now have six full and part-time cardiologists on staff, including heart failure specialists, helping to refine our program to provide care of the highest level.
We've created a Transitional Care clinic that ensures patients are seen shortly after discharge to ensure they understand and can follow a treatment plan tailored to their needs.
