What We Are Doing:
Upon admission, the RN will evaluate the patient's ability to take their medications independently. The HH health care team then initiates a plan to help the patient be as independent as possible with medication administration. Education and oversight are provided as needed. The physician is kept informed of the patients ability to manage medications competently and safely.
