|Measure: Percentage of patients who get better at getting in and out of bed|
Time Period Ending: 12/31/2012
What We Are Doing:
A patient’s ability to get in and out of bed by himself/herself is part of preparing for our day that then includes doing many other things like getting dressed or ambulating to the toilet. One of the first things we assess is the ability to get in and out of bed and if bedbound, ability to move around in bed. The Home Health team provides patients and families through tips, cues, patient/family/caregiver education and training about how to transfer safely. They also evaluate your needs for any special devices or equipment that may increase your ability to get to the safest independent level as possible.
