What We Are Doing:

Upon discharge, we provide a Congestive Heart Failure Packet covering five of the six required discharge instructions to all patients diagnosed with Heart Failure.

We have discovered that the required discharge instructions are frequently addressed with the patient, but documentation of that action is lacking. As a result, we have modified our nursing discharge process to make documentation easier.  We have also automated Heart Failure discharge instruction education to ensure the patient receives the proper information to continue proper care in the home setting.

The sixth discharge instruction (medications) is heavily driven by physician actions.  Efforts to increase our performance and accuracy in this discharge instruction include increased individual physician education, providing specific feedback to physicians, and an educational video distributed to all physician partners explaining the measure.

What can you do?

Living a healthy lifestyle is the best way to prevent heart failure. A healthy lifestyle includes exercising regularly; eating a heart-healthy diet low in saturated fat, cholesterol, and salt; losing weight if you are overweight, and quitting smoking.

Also important is carefully following your doctor's recommendations for treating conditions that may cause heart failure, such as high blood pressure, high cholesterol, and heart problems.
