What We Are Doing:

We are committed to ensuring that 100% of our patients are being assessed for appropriateness of beta blocker usage after discharge.

Our focus for improvement is to document why beta blockers may not be suitable to prescribe in every case, so a documentation reminder system is now in place. There are clinical reasons why some patients may not benefit from this treatment: for example, a lower than normal heart rate (bradycardia).

We implemented a discharge order form that will remind the physicians to order beta blockers on discharge or document a reason for not prescribing.

 What can YOU do?

Know your medications- not only the names and doses but the reason you are taking. This includes over the counter and herbals.

Get a Shared Care Plan- this is a secure and confidential place for you to organize and store all of your health information in one place. There is  a place to track allergies and medications both past and present. If you have an emergency or are admitted to the hospital health care providers can access this information.
