What We Are Doing:
This measure of clinical practice is monitored on a regular basis by doctors and staff for all of the people with diabetes who receive their care at PeaceHealth.
We have a Diabetes Wellness Assessment Program team who puts you, the patient, at the center of all we do. We want to work in partnership with you to maintain or improve your health and well being.
One of the main actions we take to meet the goal of good blood sugar control and timely health care is to send letters or make phone calls to you for regular blood work, eye exams and annual physicals.If the results of your lab tests are not where you and your healthcare provider would like them to be, then we can have a dietician work with you. Together, you can choose the steps to take to achieve your goals. You might find the staff and the dieticians encouraging some new health habits like taking your medication routinely or exercise. We do this because we know this will help lower or maintain good control of your blood sugars.
We also encourage you to attend Diabetes Education classes, even if you have previously attended. It is a good refresher on what is important to your health and most people say they hear something new every time they attend. Bring a family member or friend with you. They can be your cheerleader!
