Discharge instructions
What It MeansThe percentage of patients with heart failure that were given discharge instructions addressing all of the following: activity level, diet, discharge medications, follow-up appointment, weight monitoring, and what to do if symptoms worsen.
Why It Is Important
We know that patients and their families who understand and follow physician instructions have a lower risk of being hospitalized again for heart failure.
Numerator Definition
Patients with HF with documentation that they or their caregivers were given written discharge instructions or educational material addressing all of the following: activity level, diet, discharge medication, follow-up appointment, weight monitoring, and what to do if symptoms worsen
Denominator Definition
Patients with HF discharged to home
Denominator Excluded Populations
- Patients less than 18 years of age
