New Horizons Transitional Care Unit
Located inside Ketchikan General Hospital, the New Horizons Transitional Care Unit is a Medicare/Medicaid licensed Skilled Nursing Facility (SNF). A Transitional Care Unit? We refer to ourselves as a transitional care unit because our patients are usually on their way somewhere else: returning home, entering an assisted living facility, or receiving additional acute care. We also provide care and comfort for patients at the end of their life and assist with their Hospice care. Our Services Our services are provided in a home-like atmosphere. The rooms are bright, many with garden views and offer the amenities of home. The best part of our program is that we have all of the services you’d expect from a skilled nursing unit with the full medical resources of the hospital available to our Residents as needed. We routinely provide the following services: skilled nursing care, rehabilitation therapies, nutrition services, respiratory therapy, spiritual care, social services and activities. Features: • 29 beds • Bright residential rooms with the amenities of home • Family-style meals with self-directed meal choices • Best in Alaska for Patient satisfaction • 5-star rating for staffing level (http://www.medicare.gov/NHCompare/) • A friendly staff of RNs, LPNs, CNAs and support people • Family and visitor participation in care is encouraged Our Goal Our goal is to maximize a patient’s level of function and independence. And, we believe that the patient and family are an important part of the team. New Horizons’ Residents are our Elders. We endeavor to provide care that recognizes we all are the inheritors of their collective work, talents, experiences and traditions.
