Provider Credentialing
Welcome to the PeaceHealth Oregon Region Medical Staff Services' provider credentialing web site. Properly credentialed providers may see patients at any PHOR facility.
Getting Started
Click on the PreApplication Checklist for a list of documents required to begin the credentialing process. The checklist describes methods for submitting the required documentation (via e-mail, fax or regular mail).
The first required item is the Eligibility Criteria Statement. To retrieve that document, click on the appropriate link below.
Once you have submitted the documentation described in the PreApplication Checklist, a credentialing coordinator will contact you with additional instructions.
