You can begin the application process by downloading the application form.  

The completed application form, along with all required documents and the $350 initial application fee should be returned to the Medical Staff Services department. 

Once an application is deemed complete, it is expected to be processed within 90 days, unless it becomes incomplete. Ninety days is a benchmark because it allows extra time when verification sources do not respond in a timely manner or clarification of discrepancies.
