Requirements

• Completed application form
• Personal interview
• Background Investigation Check
• Attendance at Volunteer Orientation Class
• Organizational Integrity Training
• TB Test (provided by the hospital)
• Additional area specific training or education

Application forms may be obtained from the Information Desk at the Main Entrance to the hospital or downloaded by clicking on the link below.  Completed application forms may be mailed or returned to the Volunteer Department.  Please note that the application form may not be submitted on-line.
