Objectives

 

 

1) Address workflow to design a system that reduces waste, eliminates rework, and provides for effective, efficient use of staff resources:

  • Team-developed work processes & documentation

  • “Very Important Paper” (VIP) process

  • Nursing care teams

  • Pre-visit orientation

2) Improve communication among team members & patients:

  • George Parsons’ workshop on communication styles

  • Institute clinician – patient communication skills training

  • Providence Elderplace (PACE) site visit

3) Review GITT (http://www.gittprogram.org) approaches to team development

  • Rush GITT Interview

4) Adopt “Principles of Successful Teamwork and Team Competencies”

5) Develop and support sub-teams to address  critical pieces of the team care process: 

  • Care Conferences

  • Senior Registry/Findings

  • Patient service quality standards

  • Provider clinical quality standards

  • Diabetes wellness

6) “Transparency”

  • Internal financial/quality indicators

  • SHWC Quality Board

  • Sharing staff “Personal Accountabilities” with patients
