Steps Toward Weight Loss Surgery

1. Attend one of our free informational seminars

2. Confirm your insurance coverage for weight loss surgery (Please complete this step prior to filling out your application

a. Please call the customer service phone number on the back of your insurance card to verify the following information: 

i. Do you have weight loss surgery benefits?

1. Diagnosis code (ICD9): 278.01 morbid obesity

2. Procedure codes (CPT):

a. Laparoscopic gastric bypass: 43644
b. LapBand: 43770

ii. What criteria need to be met to be eligible for the benefit?

b. If your insurance excludes the benefit, we do offer self pay options. 

3. Complete Oregon Bariatric Center application 

a. You may bring the completed application to the informational seminar or you can mail it directly to:

b. Please include a copy of the front and back of your current insurance card(s) with your application. 

c. Once we receive your application, we will contact you within 2 weeks regarding the next step in the process. 

4. Lifestyle visits 

a. Evaluation with our surgeon 

b. Evaluation with our psychologist 

c. Evaluation with our dietitian 

d. Evaluation with our physical therapist 

e. Extended dietary program – if required by your insurance 

f. Weight loss surgery dietary education 

5. Review committee 

a. Once you have completed the above evaluations, a multidisciplinary team will meet to review your evaluations and assess your readiness for surgery. 

6. Insurance approval and preauthorization 

a. Once you have been approved by the review committee and have completed all of the necessary insurance requirements, our nurse case manager will work with your insurance company to get the surgery approved. 

7. Preoperative appointment 

a. Once you have been approved by your insurance company, we will schedule you for your preoperative appointment with the surgery. At this appointment, you will be assigned a surgery date. 

8. Preadmission testing (PAT) 

a. Two weeks prior to your surgery date, you will need to complete preadmission testing. This will be reviewed with you during your preoperative appointment.
