Why is there a Heart Failure Center?
Approximately five million Americans have heart failure, the only major cardiovascular disorder with an increasing incidence rate. The Heart Failure Center is a major initiative of the Oregon Heart & Vascular Institute to decrease Emergency Department visits and minimize hospitalizations of heart failure patients, through improving the quality of life through careful management of disease. 

What types of patients are seen in the Heart Failure Center?
The Heart Failure Center sees patients who have a diagnosis of heart failure. Patients who have symptoms that might seem like heart failure, such as fatigue, shortness of breath, or ankle swelling, may be referred for evaluation.

What services are offered at the Heart Failure Center?

  • Assessing and screening for heart failure
  • Ensuring that heart failure management blends with the management of other health conditions
  • Close monitoring and follow up
  • Ruling out other causes of symptoms
  • Adjusting medications
  • Treatment and procedures as needed for diagnosis
  • Dietary counseling 
  • Assisting with lifestyle changes 
  • Activity and exercise management
  • Self-management education 
  • Individualized follow up for each person with the goal of decreasing the symptoms of heart failure

How will my primary care doctor know what is going on with my care at the Heart Failure Center?
Frequent communication with the primary care MD/care team is an integral part of the Heart Failure Center. Communication with the health care team is a cornerstone to our success and is considered part of our framework.

How do I get an appointment at the Heart Failure Center?
Referrals come from a variety of sources, such as Primary Care, Internal Medicine, and Family Medicine physicians, as well as Cardiologists and Emergency Department physicians. If patients are interested in coming to the Heart Failure Center, they may speak with their doctors or call the Heart Failure Center directly for more information.
