Cost/Insurance Physician Referral
Medical nutrition therapy is often reimbursed by insurance providers, but that can vary with each individual insurance policy.  Please check with your insurance provider for specific coverage and benefit information. A physician referral is required for nutrition therapy.  Request that your health care provider fax a referral to our clinic at 360-715-6495.
