When you become eligible for the plan, you have 30 days from your date of hire or status change date to complete the online enrollment process. If you do not complete the process, you will default to the following benefits:

In-Network Plan - Employee Only
Core AD&D
Core Life
Core Long Term Disability
No Vision Coverage
No Dental Coverage

PeaceHealth-Oregon Region employees are benefit eligible in accordance with the following table:

Scheduled Hours Per Week	Benefit
20 or more	Medical
20 or more	Dental
20 or more	Vision
20 or more	Life Insurance
20 or more	AD & D
20 or more	Long Term Disability

Listed below are the waiting periods for benefits. A waiting period is the time period between becoming benefit eligible and the effective date benefits begin:

Medical

The first of the month following 30 days from the date you become benefit eligible.

Vision

The first of the month following the date you become benefit eligible.

Life/AD&D

The first of the month following the date you become benefit eligible.

LTD

The first of the month following 90 days from the date you become benefit eligible.

Dental

The first of the month following 90 days from the date you become benefit eligible.

Classifications of part-time and full-time jobs for benefit cost:

Part-time: 29 - 34 hours per week

Full-Time: 35 - 40 hours per week

Part-Time tier two: 20 - 28 hours per week
